Custom size wooden bases for the petite, mini, small, medium, & large size candles are available for $10.00 ea. (These are identical to our electric bases, but do not include the cord/bulb assembly)

RELEASE FORM

I understand and agree to the following:
1      To allow Design Finds Mid America, Inc. to use the best of the flowers that I, or my designee, selects for my order. (Not all flowers may fit in, or be suitable for use in, the candles.) That some flowers may dry darker than they appear in their fresh state – this is especially true with dark roses.
2      Special instructions must be outlined in the “Special Instructions” section of the order form.
3      That there will be small holes in the wax, similar to the samples shown to me, due to the handcrafted nature of the product.
4      That some greenery will be added to all candles in the production process.
5      Due to the custom nature of this product all sales are final. No returns except for freight damage.

PREPARING THE FLOWERS
Determine the total number of candles desired. (This will determine the number of flowers required). The following is a MINIMUM guideline:

         Petite, mini and small sizes          4-5 med/large flower heads per candle                                         Medium sizes                            6-9 large flower heads per candle
         Tall sizes                                  10-13 large flower heads per candle

NOTE: Including extra flower heads gives us flexibility in designing your candles. In addition to the flower heads, include several stems of greenery per candle.

Flower heads may include all varieties and sizes of roses, gerbera daisies, stargazer lilies, irises, tulips carnations, etc. Assorted colors are most popular. In most cases, flowers (if kept in a cool place and watered) will be suitable for drying if received in 5-7 days. (Spider mums do not dry well and large sunflowers are too big to use.)
a. Clip flower heads from stems – include greenery.

b. Place in UNSEALED paper bags along with a dry paper towel. Sealed bags will promote mold!

c. Label flowers (paper bag) with name of the purchaser

d. If the flowers are mailed, ship USPS Priority Mail in an appropriately-sized box (USPS
Supplies them free) to your Funeral Counselor, or to the address on the front of this order form.
